RN Case Manager - MGH

Evaluate, predict, and facilitate the trajectory of patient care by employing a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation. Meet patient needs efficiently and expeditiously to ensure institutional standards of high-quality care, reduce costs, and ensure reimbursement. Work 32 hours weekly with weekend requirements in a team environment supported by strong leaders and subject matter experts.
