Job Summary
RN Case Manager responsible for coordinating the plan of care for the patient stay, managing length of stay, ensuring appropriate resource management, and developing a safe discharge plan in collaboration with the multidisciplinary team. Drives the discharge plan, coordinates post-acute medical needs with Social Services, ensures timely referrals to social resources and payer programs, engages patient and family in setting goals, evaluates progression using InterQual criteria, facilitates patient throughput and care transitions, and advocates for patients’ rights and values while aligning needs with available resources. Strong emphasis on psychosocial assessment, care coordination, and effective communication with physicians, families, and outside agencies; NICU experience and case management knowledge are preferred.
Required Qualifications
- Registered Nurse with current TX state license required
- Associate Degree in Nursing or Nursing diploma required; Bachelor's Degree in Nursing preferred
- 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required
- RN NICU experience required with Case Management experience preferred
- Certification in case management preferred
- InterQual experience preferred
