Job Summary
RN Case Manager responsible for coordinating the plan of care, discharge planning, and ensuring patient-centered care through collaboration with the multidisciplinary team. Duties include comprehensive psychosocial and medical assessments, planning and facilitating transitions of care, coordinating post-acute needs with Social Services, making timely referrals to payer and disease-management programs, and promoting quality outcomes and efficient patient throughput while advocating for patients’ rights and needs.
Required Qualifications
- Registered Nurse with current TX state license
- Associate Degree in Nursing or Nursing diploma required; Bachelor's Degree in Nursing preferred
- 3+ years of clinical hospital nursing experience or 2+ years experience in case management
- RN NICU experience with Case Management experience preferred
- Certification in case management preferred
- InterQual experience preferred
