RN Case Manager

Job Summary
RN Case Manager coordinates the plan of care and discharge planning for patients, collaborates with multidisciplinary team to ensure appropriate resource utilization and safe transitions of care, assesses psychosocial and discharge needs, refers to Social Services when needed, aligns patient needs with available resources, communicates with physicians, families, and outside agencies, and supports adherence to regulatory requirements and quality initiatives; requires TX RN license, ADN/diploma, and 2-3+ years of relevant hospital/nursing/case-management experience; day shift with some weekend coverage in a full-time role.
Required Qualifications
- TX RN license
- Associate Degree in Nursing or Nursing diploma required
- Bachelor's Degree in Nursing preferred
- 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required
- RN NICU experience required with Case Management experience preferred
- Certification in case management preferred
- InterQual experience preferred
