Job Summary
RN Case Manager in a full-time day-shift role (8:00am–4:30pm, Monday–Friday with some weekends) at Medical City Dallas. This role involves performing comprehensive psychosocial, medical, and discharge assessments; coordinating the plan of care and discharge planning with the multidisciplinary team and Social Services; guiding care transitions; ensuring appropriate post-acute needs and resources; identifying and addressing psychosocial determinants of health; and communicating with patients, families, physicians, and outside agencies to promote quality outcomes and efficient patient throughput.
Required Qualifications
- Registered Nurse with current TX state license required
- Associate Degree in Nursing or Nursing diploma required
- Bachelor's Degree in Nursing preferred
- 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management required
- RN NICU experience required with Case Management experience preferred
- Certification in case management preferred
- InterQual experience preferred
