Job Summary
Registered Nurse Case Manager for Vitra Health in Lynn, MA. Responsibilities include completing intake and annual assessments, in-home visits, caregiver education/skills training, updating and coordinating plans of care, medication reconciliation, and communicating with clients, caregivers, and healthcare providers. Participate in multidisciplinary team meetings, ensure quality documentation, and support clients in navigating healthcare resources to remain safely in their homes. Travel within the assigned region is required; valid driver’s license and reliable transportation are needed. Requires RN licensure in Massachusetts and experience with direct care of elders or adults with disabilities. Required Qualifications
- Licensed as an RN by Massachusetts Board of Registration in Nursing, required
- Minimum of two-year recent experience in the direct care of elders or adults with disabilities
- Fluency in both English and second language strongly preferred
- Case management, behavioral health, social services and/or community health worker experience preferred
- Strong communication, documentation and organizational skills required
- Must be solution oriented and able to multitask
