Job Summary
Respond to clinical referral information with care and expertise, assessing client eligibility for specialized hospice services. Partner with the Clinical Director to guide care planning while delivering hands-on nursing support to patients and families. Educate stakeholders on personalized care goals and share call duty responsibilities to ensure continuous comfort. Maintain clinical excellence through direct patient interaction and advocacy within the interdisciplinary team.
Required Qualifications
- Hold current unencumbered license as a Registered Nurse
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Must be computer proficient in typing and various programs, including background in EMR
- Possess and maintains current CPR certification if required by state
- Current driver's license, vehicle insurance, and access to dependable transportation
