Job Summary
RN-Case Manager at Kaweah Health in Visalia, CA: Assess needs and coordinate care across the continuum, evaluate and advocate for services with physicians and multidisciplinary teams, develop and revise Care Management Plans, address Social Determinants of Health (SDOH) barriers, collaborate with patients, families, and providers to achieve goals, document findings in the electronic health record, and arrange timely referrals and transitions to appropriate post-acute and specialty care while ensuring cost-effective, quality outcomes.
Required Qualifications
- California RN license
- BLS
- Two years of acute care clinical nursing experience (LVN/RN)
- Experience in case management or discharge planning preferred
- Education Preferred: BSN or MSN or enrolled in RN-BSN program
