Job Summary
The RN Case Manager provides professional nursing services to patients aged 18 and older, coordinating care and assessing conditions. Key responsibilities include managing the care plan, educating patients and families, delegating nursing tasks, and timely documentation. Essential skills include strong communication, the ability to work autonomously in a home health setting, and proficient use of Microsoft Office software. Candidates must possess an Associate's degree in nursing and be licensed as a Registered Nurse in Missouri and Kansas, along with BLS certification.
Required Qualifications
- Registered Nurse by both Missouri and Kansas State Board of Nursing
- Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact
- Valid driver’s license and proof of insurance
- Basic level knowledge of Microsoft operating system and Microsoft Word, Excel and Outlook
- Ability to manage multiple tasks and responsibilities
- Ability to read, analyze, and interpret general business periodicals, professional journals, and technical procedures
- Ability to effectively present information and respond to questions
- Ability to handle maintenance of standards for workplace conduct
