RN-Case Manager

Coordinate and manage patient care as an RN Case Manager in home health. Responsibilities include completing comprehensive patient assessments, developing and managing individualized care plans, coordinating home health services and community resources, monitoring patient progress, collaborating with physicians, therapists, caregivers, and family members, educating patients and families on health management, and maintaining accurate clinical documentation while ensuring compliance with regulatory standards.
