RN Case Manager

Conduct in-person patient interviews and comprehensive physical assessments, then oversee the implementation and ongoing assessment of patient care plans by managing home health aides, LPNs, RNs, and other caregivers. Communicate patient conditions to collaborate with providers as needs evolve, while providing education on procedures like wound care, IV administration, and medication management. Work to decrease readmissions by promoting preventative care and ensuring continuity. Requires current unrestricted RN licensure, CPR/AED/BLS/First Aid certification, one year of home health experience, and reliable transportation.
