RN Case Manager

Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Assess and document patient conditions, administer medications including insulin and IV fluids, and inspect wounds to deliver appropriate treatments. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. Current unrestricted RN licensure, an ADN or AAS degree, and CPR/AED/BLS certification are required. Competitive pay, benefits, incentives, and flexible scheduling are offered.
