Job Summary
RN Case Manager in inpatient setting responsible for coordinating patient care across the care continuum, collaborating with physicians and an inter-professional team to plan and facilitate discharge needs, and linking patients to post-acute resources. Applies nursing process to assess, diagnose, plan, implement, and evaluate care; provides patient/family education and advocacy; aims to improve clinical outcomes, ensure appropriate level of care during hospitalization and after discharge, and support efficient utilization of health services. Requires discharge-planning education, competency in nursing practice, and adherence to applicable nursing standards and ethics.
Required Qualifications
- Graduate from an accredited nursing program (CCNE/ACEN/NLN CNEA accreditation acceptable).
- Current unencumbered RN license in the practicing state or multistate license (NLC).
- BLS certification within 60 days of hire.
- Bachelor’s degree in Nursing strongly preferred in Bemidji and Rapid City; required in Fargo and Sioux Falls.
- If applicable, leadership may consider an educational plan to achieve a BSN within five years of hire.
- CCM certification encouraged when eligible.
