Job Summary
Collaborate with licensed practitioners and inter-professional team members to provide care coordination, transitional care planning, and facilitation of discharge needs for assigned patient populations. Utilize the nursing process to deliver individualized nursing care, including patient assessment, triage, and education for patients, caregivers, and families. Anticipate educational needs and expedite early discharge planning by initiating links with post-acute external care resources while advocating for patients across the continuum of care. Manage resource allocation to enhance clinical outcomes and patient satisfaction while controlling costs. Requires a Bachelor's degree in Nursing (preferred in most markets) and an unencumbered RN license.
Required Qualifications
- Bachelor's degree in Nursing
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
- Incumbent must obtain and subsequently maintain required department specific competencies and certifications including Basic Life Support (BLS) within 60 days of hire
Desired Qualifications
- Bachelor's degree in Nursing strongly preferred in Bemidji, Wisconsin, Michigan and Rapid City
- Minimum of two years' clinical experience preferred
- Certification in: Case Management (CCM), or holds a certification in their patient population specialty that they serve, is encouraged when eligible
