RN Case Manager, Inpatient Cardiology

Job Summary
Assess, plan, and oversee daily clinical operations for care coordination and transitions to address the comprehensive health needs of pediatric patients and families. Collaborate with clinical teams to obtain medically necessary equipment, including intravenous therapies, durable medical equipment, and supplies, while determining options based on clinical need and insurance constraints. Perform clinical assessments for skilled nursing, home health aides, and personal care attendants to ensure safe discharge. Integrate regulatory requirements, resource allocation, and funding sources to optimize service delivery within the Cardiology multidisciplinary team. Participate in hospital and departmental programs and committees as required. Work 40 hours per week, Monday through Friday, from 8:00 am to 4:30 pm.
Required Qualifications
- A Bachelor's degree in nursing
- A minimum of 5 years nursing experience
- Current Massachusetts license as a Registered Nurse (RN)
Desired Qualifications
- Master's in a related Healthcare field
- pediatrics
- Experience as a Case Manager (Discharge Planning and/or Utilization Management)
- A certification as a Certified Case Manager
- willingness to obtain [a certification as a Certified Case Manager] within the first two years of employment
