RN Case Manager, Inpatient Cardiology

Job Summary
Assess, plan, organize, and oversee daily clinical operations for care coordination and transitions to address complex health needs of patients and families. Collaborate with inpatient nursing teams and multidisciplinary groups to obtain medically necessary equipment and services for safe discharge, including intravenous therapies, durable medical equipment, and home health aides. Integrate regulatory requirements, resource allocation, and funding sources to optimize service delivery while considering fiscally responsible options. Work 40 hours per week, Monday through Friday, from 8:00 am to 4:30 pm with rotating holidays and weekends.
Required Qualifications
- A Bachelor's degree in nursing
- A minimum of 5 years nursing experience
- Current Massachusetts license as a Registered Nurse (RN)
Desired Qualifications
- Master's in a related Healthcare field
- pediatrics
- Experience as a Case Manager (Discharge Planning and/or Utilization Management)
- A certification as a Certified Case Manager
- willingness to obtain [a certification as a Certified Case Manager] within the first two years of employment
