RN Case Manager

Job Summary
Coordinate home care services for patient visits while supervising CNAs and PRN nurses. Manage care coordination between disciplines including PT, OT, ST, CNA, and MSW. Ensure timely and accurate documentation while maintaining accountability, professionalism, and confidentiality. Lead team responsibilities and communicate effectively within a family-friendly, team-oriented environment. Requires current NC RN license, one year of experience in home care or hospice, and a valid NC driver's license. Occasional weekend rotation and on-call duties apply. Day shift from Monday to Friday with a minimum of 32 hours per week.

- Must have graduated from an accredited school of nursing
- Must have a current RN license to practice in NC
- Must have at least one year of experience as an RN in a hospital/home care/hospice/public health setting
- Valid NC driver's license
- Reliable transportation
