Hospice RN Case Manager in Twin Falls, ID responsible for managing a field-based patient caseload across the Twin Falls/Magic Valley service area, performing home visits, assessing symptoms, coordinating medications and care-plans, educating patients and families, and collaborating with the interdisciplinary hospice team; requires Idaho RN license (or compact) and at least 1 year of post-graduate RN experience; travel locally with on-call rotations and a full-time schedule; compensation $80,000-$97,000 with benefits.
Required Qualifications
- Current Idaho RN license or valid compact RN license
- Clear path to Idaho RN licensure before start
- At least 1 year of post-graduation RN experience
- Hospice, palliative care, home health, oncology, ICU, ER, med-surg, skilled nursing, long-term care, or another clinically intensive RN background
