Job Summary
Provide skilled palliative and end-of-life care in the community setting as a Case Manager RN, coordinating an interdisciplinary plan and ensuring medical management of hospice services for patients and families. Requires independent clinical assessment, ability to work within an interdisciplinary team, adherence to state/federal home care guidelines, and maintaining licensure/certifications. Duties include planning, organizing, prioritizing work, and applying strong communication to patients and families. Requires driving as part of home care delivery; shift is full-time, days, Mon-Fri, with potential weekend coverage; 40 hours weekly; non-exempt. BLS and California RN licensure are mandatory; valid driver’s license and auto-insurance are required.
Required Qualifications
- RN license in California
- Graduate of an accredited School of Nursing
- BLS Healthcare Provider
- Valid Driver's License
