Job Summary
Collaborate with healthcare professionals to design and implement care plans for in-home hospice patients while maintaining meticulous records. Act as an advocate for patients and their families, ensuring they receive compassionate and comprehensive support during their most critical times. Work independently and as part of a multidisciplinary team to deliver high-quality care across the Greater Denver area. Requires a valid Colorado RN license, minimum one year of hospice experience, and reliable transportation for field visits.
Required Qualifications
- A valid RN license in Colorado
