RN Case Manager - Hospice

Complete initial and ongoing comprehensive assessments to determine hospice needs and develop individualized care plans with patients and families. Administer medications and treatments as prescribed while educating caregivers on care techniques, symptom management, and diet. Lead meaningful end-of-life conversations to ensure exceptional symptom management with empathy and clinical excellence. Rotate on-call duties to ensure seamless care transitions. Requires 1+ years of acute care experience, a valid driver's license, and reliable transportation. Full-time schedule from 8:00am to 5:00pm with benefits including 401(k) match, tuition reimbursement, and paid time off.
