Job Summary
RN Case Manager to develop and implement individualized hospice care plans, deliver one-on-one and family-centered care, monitor vital signs, administer physician-ordered medications and treatments, educate patients and families on care techniques and symptom management, track changes in patient condition with thorough clinical notes, and collaborate with interdisciplinary teams to ensure compassionate end-of-life care; requires active RN license in the state of work, CPR certification, valid driver’s license, and strong communication skills.
Required Qualifications
- Active Registered Nurse license in good standing in the state you will work in

- A valid driver’s license and reliable transportation
