Job Summary
RN Case Manager in Hospice with a focus on coordinating interdisciplinary hospice care for patients and families. Leads care planning, conducts comprehensive nursing assessments, and delivers effective pain and symptom management. Collaborates with the care team to ensure patient safety, smooth transitions between care settings, and thorough patient/family education. Requires RN licensure, at least one year of recent medical/surgical or critical care nursing experience, strong assessment and communication skills, and a commitment to professional conduct within hospice care. Location: Albuquerque, NM; Schedule: full-time with on-call responsibilities.
Required Qualifications
- Graduate of accredited school of nursing
- Current RN licensure in good standing
- Minimum one year of recent experience in medical, surgical or critical care as a professional nurse
- Strong clinical assessment skills
- Excellent communication and interpersonal skills
- Ability to develop and maintain therapeutic relationships
- Ability to conduct comprehensive nursing assessments
- Ability to plan care and intervene effectively
- Ability to provide pain/symptom assessment and management
- Commitment to patient safety
- Ability to facilitate smooth transitions between care environments
- Ability to provide patient/family/caregiver teaching
- Ability to collaborate in providing patient care
