RN Case Manager (Hospice) PRN

Job Summary
Provide and coordinate hospice nursing care as an RN Case Manager, including implementing the patient care plan (POC), documenting patient status, collaborating with the attending physician and IDT, instructing patient/family, supervising hospice aides, and conducting regular supervisory visits and annual evaluations. Requires an active Louisiana RN license, two years of full-time RN experience (hospital/hospice experience acceptable via LPN substitution), on-call participation, and compliance with Medicare/Medicaid requirements. Job types include Full-time, Part-time, and PRN. Benefits available including health, retirement, and paid time off.
Required Qualifications
- licensed registered nurse must be currently licensed to practice in the state of Louisiana with no restrictions
- With at least two years FULL time experience as a registered nurse. However, two years of full time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse
- Be an employee of the hospice. If the registered nurse is employed by more than one agency, he or she must inform all employers and coordinate duties to assure qualified service provision.
- Annual continuing education: at least two hours related to end of life care
- Maintains eligibility to participate in Medicare and Medicaid programs throughout employment
- Capable of providing emergency care as needed for any patient
- Participate in pre-employment and annual Tuberculosis screening
