Job Summary
RN Case Manager (Hospice) coordinates the implementation of the patient care plan (POC) for hospice patients, identifies physical, psychosocial, and environmental needs, documents and communicates patient status, collaborates with the attending physician and IDT, educates patients/families on self-care, supervises hospice aides and other staff, conducts supervisory visits to patient residences, and maintains compliance with licensure and ongoing education requirements. Requires active RN license in Louisiana, at least two years of full-time RN experience (hospice experience acceptable via LPN substitution), ability to document and submit clinical notes within one week of visits, and participation in annual TB screening and disaster/evacuation readiness. Required Qualifications
- Licensed RN in Louisiana
- RN License (Required)
- At least two years full-time experience as RN with hospice experience preferred
- Continuing education—two hours annually in end-of-life care
