RN Case Manager (Hospice) - Part Time

Admit, assess, discharge, and pronounce patients while coordinating appropriate care for each patient and family as an integral part of the interdisciplinary care team. Manage admissions, assessments, discharges, and pronouncements for out-patient hospice cases in the Clear Lake area. Requires a State of Texas RN license with one year of RN experience, plus preferred hospice or home health background.
