Job Summary
Provide skilled nursing assessments, planning, and care within the interdisciplinary team for palliative/hospice patients; deliver education and emotional support to patients and families; develop and update treatment plans; utilize EMR for documentation; ensure HIPAA compliance; coordinate care with IDT, facilities, and other health care providers; offer on-call nursing support and home health visits as part of comprehensive end-of-life care.
Required Qualifications
- Possess valid California RN license
- Graduate of an accredited school of nursing in California
- Three years of recent clinical experience in nursing
- Ability to use computer systems and EMR
- Knowledge of palliative care nursing, crisis intervention, and family dynamics
- Ability to work in high-stress environment and manage multiple tasks
- Reliable transportation with California driver's license and automobile insurance
