Job Summary
Chart patients and manage care plans for medical-surgical, homecare, and hospice populations while assessing limits of intervention and prioritizing care. Provide basic and skilled nursing care, utilizing clinical expertise in critical thinking, assessment, and problem-solving. Communicate effectively with patients, families, and interdisciplinary team members regarding care issues. Maintain documentation in an EMR system, with Netsmart preferred, and ensure compliance with Ohio and Michigan licensing standards. Must be a licensed driver with a valid automobile and hold current CPR certification. Requires strong organizational skills and the ability to work independently within a team.
Required Qualifications
- Graduate of an accredited school of registered nursing
- Currently licensed as an RN through the Ohio Board of Nursing
- Must be able to obtain a license in the state of Michigan
- Has working experience charting in an EMR system
- Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills
- Possesses the ability to provide basic and skilled nursing care
- Possesses the ability to set priorities of care
- Possesses the ability to appropriately assess limits of intervention
- Possesses strong verbal and written communication skills
- Demonstrates ability to communicate effectively with patients, families, and team members regarding care issues
- Has the ability to work independently and within an interdisciplinary team
- Demonstrates strong organizational, time management, and prioritization skills
- Must be a licensed driver
- Must have an automobile in good working condition
- Must be insured in accordance with state and agency requirements
- Understands the hospice philosophy
- Understands principles of death/dying
- Understands concepts of pain/symptom management
- Understands adult teaching/learning principles
- Meets state requirements for maintaining professional license
- Must be CPR certified according to the American Heart Association CPR standards
Desired Qualifications
- BSN
- Minimum 2 years of medical-surgical nursing experience
- Previous experience in homecare
- Previous experience in hospice
- Experience with Netsmart system
