Job Summary
Hospice RN Case Manager to manage a dedicated hospice caseload in IL with a Monday–Friday schedule (8:30 AM–5:00 PM), one weekend rotation every 12 weeks, and a defined territory (Lake Forest, Waukegan, Zion). Compensation ranges from $80,000 to $97,000 annually based on experience, with comprehensive benefits including 401(k) match, health insurance, PTO, mileage reimbursement, tuition reimbursement, hospice certification support, and merit-based raises. Responsibilities include patient assessments, admissions, individualized care planning, delivering skilled nursing care focused on comfort and dignity, coordinating with physicians and the interdisciplinary team, routine visits with care-plan adjustments, end-of-life education for families, timely documentation, and supervising Home Health Aides in compliance with regulations.
Required Qualifications
- Graduate of an accredited RN program
- Active Illinois RN license
- Current CPR and First Aid certification
- Hospice or home health experience preferred
- Strong clinical judgment and communication skills
- Valid driver’s license and proof of insurance
