RN-Case Manager

Job Summary
Assess client health needs by developing and implementing Plans of Care based on clinical assessments, diagnosis, and utilization guidelines. Monitor core measurements, administer medications, and coordinate care with physicians and interdisciplinary teams to ensure compliance with OSHA, JCAHO, and federal standards. Document admissions, progress, and discharge planning accurately using OASIS data collection while supervising LPNs and Home Health Aides. Participate in backup calls, interdisciplinary conferences, and accreditation programs to promote positive patient outcomes. Requires current Florida RN license, CPR certification, and driver's license.
Required Qualifications
- Current RN license in the State of Florida
- Current CPR certification
- Current driver's license and valid auto insurance
- Completion of 25 CEU's bi-annually
- Knowledge of OSHA, JCAHO/CHAP, State and Federal standards
- Familiarity with payor reimbursement guidelines
- Knowledge of standard precautions
- Must thoroughly understand PPS
- Must be clinically astute
- Must utilize case management guidelines
- Must utilize automation effectively
- Must incorporate effective processes and collaboration
- Must be able to lift 50 lbs
Desired Qualifications
- Familiarity with decimals and metric system
- Proficient in use of OASIS
- Good communication skills
