RN Case Manager - Hospice

Job Summary
Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs, managing pain and physical symptoms. Initiate and develop plans of care in collaboration with the Interdisciplinary Group, coordinating services among healthcare providers to guide care delivery. Administer prescribed medications and treatments while providing education and interventions for symptom management. Supervise LPNs and Hospice Aides assigned to patients and complete accurate, timely documentation of all services provided. Regularly attend Interdisciplinary Group meetings to ensure continuous assessment of needs. Visit patients one-on-one in their home environments within the southwest Twin Cities Metro area.
Required Qualifications
- Licensed to practice as an RN in the state of MN. License is active and in good standing.
- Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate.
- Able to travel within a geographic area serviced by the office.
- Reliable transportation.
- Valid drivers license.
- Proof of auto insurance is required.
- CPR Certification
- All offers of employment are contingent upon successful completion of a criminal background check, reference checks, and drug screening.
Desired Qualifications
- One (1) year of hospice care experience preferred.
