RN Case Manager - Hospice

Job Summary
Provide admissions and nursing care to hospice patients in their homes; coordinate with Interdisciplinary Group/Team to assess and address physical, psychosocial, and environmental needs. Utilize Nursing Assessment and LCD criteria to develop and document care plans, perform nursing care with infection control, and communicate with hospice physicians and other health professionals. Submit required paperwork in a timely manner, participate in hospice in-service programs and the QAPI program, promote the hospice philosophy, maintain productivity, and provide on-call services. Travel to patient homes and office/support locations as needed; maintain effective communication with patients, families, and care team; comply with regulatory standards and documentation requirements. Requires reliable transportation, ability to travel locally, and a valid auto liability insurance policy.
Required Qualifications
- Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Licensure: Current State license as a Registered Nurse; Current driver’s license
- Skills: Nursing skills as defined as generally accepted standards of practice
- Knowledge of state and federal regulations
- Good interpersonal skills
