RN Case Manager - Hospice

RN Case Manager coordinates skilled nursing care for homebound patients in their residence, administers nursing care, collaborates with the interdisciplinary team and family, conducts comprehensive assessments (including ongoing reassessments and plan of care updates), initiates and revises care plans, provides education, and ensures continuity of care. The role requires home health nursing skills, ability to travel locally, manage on-call responsibilities, documentation of progress notes, and supervision of other nursing personnel. Must maintain CPR and current nursing license; requires reliable transportation and a flexible schedule; environment includes home settings with potential exposure to bodily fluids. Work location is in-person in Mishawaka, IN.
