RN Case Manager (Hospice) FT

Job Summary
RN Case Manager for Hospice: coordinate implementation of the patient’s plan of care, provide nursing services, document clinical notes within one week, collaborate with the patient/family and IDT, instruct on self-care, supervise hospice aides and ancillary staff, conduct supervisory visits to patient residences biweekly, perform annual supervisory activities for aides and LPNs, participate in on-call rotation, and maintain licensure and continuing education in end-of-life care. Requires Louisiana RN licensure, two years of full-time nursing experience ( hospice experience acceptable in lieu of RN years), and employment as hospice staff with ongoing Medicare/Medicaid eligibility. Required Qualifications
- licensed registered nurse (RN) in Louisiana with no restrictions
- minimum two years full-time nursing experience; hospice clinical experience acceptable in lieu of RN experience
- employment as an employee of the hospice; coordination of duties if employed by multiple agencies
- annual continuing education: at least two hours related to end-of-life care
- not excluded from Medicare/Medicaid participation; maintain eligibility
- ability to provide emergency care and participate in disaster/evacuation situations
- pre-employment and annual tuberculosis screening
