RN Case Manager (Hospice) FT

Job Summary
Coordinate the implementation of the Plan of Care (POC) for each patient by identifying physical, psychosocial, and environmental needs and reassessing them at least every 14 days. Provide nursing services in accordance with the POC, document goals and interventions, and collaborate with the attending physician and interdisciplinary team. Instruct families in self-care techniques, supervise ancillary personnel, and conduct supervisory visits to residences every 14 days to evaluate aide performance and patient relationships. Complete accurate clinical notes within one week of visits and prepare written instructions for personal care services. Maintain eligibility for Medicare and Medicaid, obtain annual continuing education, and participate in emergency response and TB screening.
Required Qualifications
- licensed registered nurse
- currently licensed to practice in the state of Louisiana with no restrictions
- at least two years FULL time experience as a registered nurse
- two years of full time clinical experience in hospice care as a licensed practical nurse
- employee of the hospice
- annual at least two hours of continuing education hours related to end of life care
- not been excluded from participating in Medicare and Medicaid programs
- eligibility to participate in Medicare and Medicaid programs throughout employment
- Capable of providing emergency care as needed for any patient
- Maintains a neat, well groomed appearance at all times
- Keeps personal appearance appropriate for patient care
- Ability and willingness to participate appropriately in a disaster or evacuation situation
- Participate in pre-employment and annual Tuberculosis screening
- RN License
