Job Summary
RN Case Manager Hospice: Provide end-of-life hospice nursing care, assess patients and families, and develop individualized care plans. Lead admission facilitation, clinical support, and continuous care for terminally ill patients in home or community settings. Coordinate with an interdisciplinary team, conduct after-hours calls, document patient problems, goals, and outcomes, and present information for bi-weekly IDG meetings. Train and support patients, families, and caregivers while ensuring quality end-of-life care.
Required Qualifications
- Graduate of accredited school of nursing
- Current Minnesota RN licensure
- Three years recent nursing experience in acute,long-term care or home care/hospice/public health
- Patient care planning and patient teaching skills
- Critical thinker with excellent written and verbal communication skills
- Current Minnesota driver’s license, insurance and daily access to reliable vehicle
