Job Summary
Conduct comprehensive patient assessments to develop individualized hospice care plans and coordinate interdisciplinary team meetings to review and update patient goals. Monitor patient conditions regularly, adjust care plans as necessary, and provide education and support regarding disease progression and symptom management. Serve as the primary point of contact for patients, families, and healthcare providers to facilitate communication, ensure compliance with regulations, and advocate for patient rights and preferences. Manage multiple cases while maintaining accurate documentation and collaborating with community resources to enhance support. This full-time role at Blue Summit Hospice and Palliative Care in Snellville focuses on delivering compassionate, patient-centered end-of-life care to individuals facing life-limiting illnesses.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the state of Georgia
- Minimum of two years of clinical nursing experience
- Strong knowledge of hospice care principles, symptom management, and end-of-life care practices
- Excellent communication and interpersonal skills
- Ability to manage multiple cases simultaneously while maintaining attention to detail and documentation accuracy
Desired Qualifications
- Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification
- Experience with electronic medical records (EMR) systems and hospice-specific documentation software
- Background in case management or care coordination within a hospice or home health setting
- Familiarity with Medicare hospice regulations and reimbursement processes
