Job Summary
RN Case Manager will coordinate hospice care for patients and families, conduct comprehensive assessments, develop and adjust care plans with physicians and care teams, administer medications per orders, maintain accurate documentation, serve as liaison among families, physicians, and community health partners, educate patients and caregivers, and participate in on-call rotation.
Required Qualifications
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing
