RN Case Manager- Hospice

Job Summary
Assess hospice care patients and families to identify physical, psychosocial, and environmental needs using documentation, clinical records, interdisciplinary reports, and on-site evaluations. Communicate significant findings, problems, and changes in health conditions or unsafe facility conditions to the clinical supervisor, physician, and other personnel involved with patient care. Counsel, instruct, and include patients, facilities, and families in following the interdisciplinary plan of care while teaching hospice philosophy and pain management to staff in alternate care settings. Provide appropriate support at time of death and perform bereavement assessments. Ensure all activities align with the vision of the board of directors and hospice leadership. Requires a current RN license, one year of clinical experience, and 50% travel.
Required Qualifications
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver's license and insurance and reliable transportation to perform job tasks
