RN Case Manager - Hospice Augusta - Martinez, GA, Days

Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance to increase productivity and quality of care while adhering to hospice policies and goals. Develop patient Plans of care in collaboration with other professionals, implementing treatment strategies that promote physical, psychosocial, emotional, and spiritual well-being. Maintain patient confidentiality and exercise sound critical decision-making in a cost-effective manner. Requires Georgia RN licensure, one year of clinical experience, one year of hospice experience, and valid CPR and driver's license.
