Job Summary
Conduct comprehensive physical, social, and psychological assessments while collaborating with interdisciplinary teams to develop holistic care strategies. Coordinate patient care delivery, manage resources, and maintain meticulous patient records through full-range patient transfers. Initiate referrals and engage families in meaningful ways to ensure personalized solutions for hospice patients. Requires current Nebraska RN license, hospice experience, and valid driver's license with clean record.
Required Qualifications
- Current Registered Nurse (RN) license in Nebraska
- Hospice experience as a Registered Nurse
- Valid NE Nursing License
- Valid Driver's License (clean record) and reliable transportation
- Proof of current auto insurance
- Current CPR Certification
- Negative TB Test
- Must be able to pass a criminal background check
