Job Summary
Hospice RN Case Manager manages a dedicated hospice caseload with patient assessments, admissions, and individualized care planning. Delivers skilled nursing care focused on comfort and dignity, coordinates care with physicians and interdisciplinary team members, conducts routine visits and adjusts care plans as needed, provides end-of-life education to families, maintains timely documentation, and supervises Home Health Aides in compliance with regulations. Requires an Indiana RN? (Note: license is Illinois), active RN license, CPR/First Aid certification, and hospice or home health experience. Territory includes Lake Forest, Waukegan, and Zion, IL; schedule is Monday–Friday 8:30 AM–5:00 PM with limited weekend rotations; compensation $80,000–$97,000/year plus benefits.
Required Qualifications
- Graduate of an accredited RN program
- Active Illinois RN license
- Current CPR and First Aid certification
- Hospice or home health experience preferred
- Strong clinical judgment and communication skills
- Valid driver’s license and proof of insurance
