Job Summary
Hospice RN Case Manager providing patient-centered end-of-life care in the Northbrook, IL area. Manage a dedicated hospice caseload, perform patient assessments and admissions, and develop individualized care plans. Deliver skilled nursing care focused on comfort and dignity; coordinate care with physicians and interdisciplinary team members; conduct routine visits and adjust plans as needed. Provide end-of-life support including family education and pronouncement; supervise Home Health Aides in compliance with regulations; maintain timely, accurate documentation. Requires an active Illinois RN license, graduation from an accredited RN program, CPR/First Aid, and driver’s license. Hospice or home health experience preferred.
