Job Summary
RN Case Manager in Homecare for Agrace in Waukesha. Full-time day shift with on-call rotation (weekends and holidays) and a $6,000 sign-on bonus. Responsible for patient and family-centered nursing care and case management, including assessments, developing and updating the plan of care, delivering interventions, providing education, and documenting outcomes. Coordinates with the Interdisciplinary Team to determine hospice eligibility, oversees CNA/LPN activities, participates in meetings, and precepts new staff. Strong emphasis on compassionate, patient-centered care and adherence to policies and regulatory standards.
Required Qualifications
- ADN or BSN required
- Current, unrestricted Registered Nursing License in state of employment
- One-year nursing experience preferred
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Must be at least 18 years of age
- Basic Microsoft Office Skills
- Intermediate electronic health records skills
