RN Case Manager - Home Hospice

Job Summary
Conduct thorough patient assessments to evaluate physical, emotional, and psychosocial needs in the home environment. Develop, implement, and regularly update individualized care plans in collaboration with interdisciplinary hospice teams. Coordinate communication with physicians, social workers, chaplains, and other healthcare professionals to optimize patient care and manage symptom control. Provide education and support to patients and families regarding disease progression and available resources while ensuring compliance with hospice regulations and documentation standards. Facilitate timely admissions, transfers, and discharges to maintain continuity of care. Serve as a patient advocate, respecting cultural, spiritual, and personal preferences throughout the hospice experience.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the United States
- Minimum of two years of clinical nursing experience
- Ability to work independently in a home care setting
- Excellent organizational and time management skills
- Proficiency in electronic medical records (EMR) and documentation practices
- Strong communication skills
