Job Summary
RN Case Manager - Home Hospice coordinates patient care across Northwest Arkansas and NE Oklahoma. Responsibilities include performing comprehensive physical and pain assessments, managing symptoms related to terminal illness, coordinating Circle of Life services for patients and families, completing and maintaining documentation (assessments, care plans, clinical notes, agency tracking forms), establishing therapeutic nurse-patient relationships, providing weekend coverage, and assisting during nursing shortages. The role requires communicating with attending physicians, staff, and other agencies to optimize care and resources, educating patients and caregivers about hospice philosophy and disease progression, updating the Plan of Care (POC) at least every 60 days per regulatory standards, and participating in IDT meetings. Additional duties include arranging CNA supervision and services, revising CNA care plans as needed, ensuring confidentiality and compliance with policies and regulatory requirements, and promoting compassionate community care. Employment is with Circle of Life, Northwest Arkansas’ premier not-for-profit hospice provider, serving Benton, Washington, Madison, and Carroll counties with two inpatient facilities and outpatient services. Required Qualifications
- Current unrestricted AR RN license
- BLS certification
