RN Case Manager - Home Hospice

RN Case Manager – Home Hospice manages patient care across assigned territories with comprehensive assessments, pain and symptom management, and coordination of Circle of Life services; responsibilities include documenting assessments and care plans, maintaining therapeutic nurse-patient relationships, scheduling weekend coverage, and participating in IDT meetings to update the plan of care. Requires an active Arkansas RN license and BLS, strong assessment and documentation abilities, and experience in hospice/home health or acute care nursing with computer charting. Must communicate with physicians and care teams, educate patients/caregivers on hospice philosophy and disease progression, and adhere to regulatory standards while demonstrating compassion and efficient use of resources. Locations include Springdale, AR (two entries reflecting same locale).
