RN Case Manager - Home Hospice

Job Summary
Conduct comprehensive physical and pain assessments, manage symptom control for terminal illness, and coordinate Circle of Life services with patients and families. Establish therapeutic nursing relationships, document all care plans and clinical notes per policy, and communicate with physicians and agencies to optimize resource use. Educate caregivers on disease progression and hospice philosophy, review patient care plans every 60 days, and supervise Certified Nursing Assistants. Identify patients needing discharge or service changes, maintain annual clinical competencies, and participate in interdisciplinary team meetings to ensure care reflects current needs. Work M-F 8-4:30 across NWA and NE Oklahoma territories, providing weekend coverage when necessary.
Required Qualifications
- Current unrestricted AR RN license
- BLS certification
- Strong assessment skills
- Critical thinking
- High level of problem solving
- Strong documentation skills
- Communication skills
- Ability to stay calm
- Ability to cope with human suffering/emergencies
- Computer charting experience
- Prefer hospice experience
- Prefer home health experience
- Prefer acute care nursing experience
