RN Case Manager - Home Hospice

Job Summary
Conduct comprehensive physical and pain assessments, manage symptom control for terminal illness, and coordinate Circle of Life services with patients and families. Establish therapeutic nursing relationships while completing required documentation, care plans, and clinical notes in accordance with agency policies. Provide weekend coverage and assist during nursing absenteeism, communicate with physicians and agencies to optimize care, and educate caregivers on hospice philosophy and disease progression. Review and update patient care plans every 60 days, participate in interdisciplinary team meetings, assess needs for CNA services, and identify patients for discharge or revocation. Maintain annual clinical competencies, adhere to all regulatory standards, and model prompt attendance while promoting respect for patient privacy and confidentiality.
Required Qualifications
- Current unrestricted AR RN license
- BLS certification
- Strong assessment skills
- Critical thinking
- High level of problem solving
- Strong documentation skills
- Communication skills
- Ability to stay calm
- Ability to cope with human suffering/emergencies
- Computer charting experience
- Prefer hospice experience
- Prefer home health experience
- Prefer acute care nursing experience
