Job Summary
Oversee coordinated care delivery and supervise clinical activities for assigned patients and staff. Provide accurate documentation to ensure patient reauthorization and deliver services in the field, including in-home visits and attending physician appointments. Implement and update Plans of Care while conducting orientations for new staff and ensuring timely completion of chart reviews and quality assurance documentation. Establish collaborative relationships with patients, physicians, payers, and healthcare teams to support optimal clinical and operational outcomes. Participate in quality assurance committees and state survey preparation as needed.
Required Qualifications
- Associate's or Bachelor's degree in a related field
- Current Registered Nurse (RN) license to practice professional nursing in each state where patient care is supervised
- Current Basic Cardiac Life Support and CPR certification
- At least three (3) years of experience as a Registered Nurse
- At least one (1) year of case management experience
- Maintain current state licensure
- Maintain current liability and malpractice insurance, if applicable
- Valid Driver's License
- Active RN License
- Criminal background check clearance
- Driving Record clearance
- OIG Exclusion List clearance
- Sex Offender Registry clearance
